Upon request you will be provided with an itemized superbill to submit to your insurance provider for potential reimbursement. This document contains the necessary codes and details required by insurers. Please check with your insurance provider for any additional forms they may require.

Understanding Insurance Coverage
Insurance coverage for lactation services varies widely. It is the client’s responsibility to verify coverage and understand their plan’s out-of-network reimbursement process. Contacting member services can provide clarity on specific steps required for reimbursement, as each insurer handles claims differently.
Please note that Luxe Lactation LLC does not submit claims on your behalf. Insurance reimbursement is the responsibility of the client.

All fees are due at the time of your appointment unless alternate arrangements have been made in advance. Payment can be made via:
